Admission Requirements:

On admission you will need to complete an admission form.  You will be asked to provide us with  personal info, health history, past surgeries and chronic medication information.

​

Our healthcare teams are there to listen to your concerns and personal needs. The teams will answer any of your questions to the best of their ability.

​

We will provide a checklist for your convenience that should assist you to prepare for your admission, (also see below pamphlet for operations).

What you'll need:

1. Medical Aid Card;

2. Medical Aid Number;

3. Authorisation Number;

4. Identity Document of Patient and Main Member;

5. Referral letter from your Doctor
